How it works
Forms in GoHighLevel
Forms embed on any website in four layouts, inline, popup, slide-in or sticky sidebar, with three trigger types and three deactivation rules. The thing that makes them worth using over a dedicated form tool is not the builder, which is ordinary. It is that every submission arrives as a contact in the CRM with workflows already able to act on it.
Research-led reviewThe author has not used this product. Built from public sources. What this means
The author has not used this product. Everything here comes from six public sources: the maker's own site, its pricing and refund terms, its changelog, its support forum, counted complaints across recent public reviews, and a like-for-like comparison against the nearest alternatives. We never claim experience we do not have.
Which badge a page carries is enforced in the site build, not just in editing. A page cannot use first-hand language unless it has declared itself a real-use review, so the badge and the writing cannot drift apart. The full method

You pay exactly what you would pay going direct. The maker pays us a share of their margin, not a surcharge on you.
We are never paid for a positive verdict, no maker sees a review before it publishes, and we publish verdicts recommending against products we could earn from. Commission rates never influence which products we cover or how we rank them. Full disclosure
The short version
- Forms, surveys and quizzes are three tools with one builder and different jobs.
- They embed on non-HighLevel sites, so a WordPress site can feed the CRM directly.
- Conditional logic is included at every tier, not held back as a paid feature.
- Custom CSS is supported, which is the escape hatch when the styling options run out.

Forms, surveys and quizzes
| Tool | Shape | Use it for |
|---|---|---|
| Form | Everything on one screen | Contact requests, quote requests, anything short |
| Survey | One question or step at a time | Qualification, onboarding, anything long enough that a single page would scare people off |
| Quiz | Questions with scored outcomes | Lead scoring and segmentation dressed up as something the visitor enjoys |
The distinction that matters is between forms and surveys, and it is about completion rather than features. A twelve field form shown all at once looks like work and people leave. The same twelve fields presented three at a time, with progress visible, consistently finish better. If you are asking for more than about six things, build a survey.
Embedding anywhere
| Layout | Behaviour |
|---|---|
| Inline | Sits in the page content as a native element |
| Popup | A modal over the page |
| Polite Slide-In | Slides in from the left or right |
| Sticky Sidebar | Pinned to the left or right of the screen |
All four work on sites that are not built in HighLevel, which is the point people miss. A business with a WordPress site it has no intention of replacing can still put a HighLevel form on it, and every submission lands in the CRM with workflows able to fire immediately. You do not have to migrate the website to start using the CRM.
| Show it | Stop showing it |
|---|---|
| Show on scrolling X per cent | Deactivate after showing X times |
| Show after X seconds | Deactivate once a lead is collected |
| Always show | Never deactivate |
Conditional logic
Conditional logic is available in both forms and surveys, and it is included at every tier rather than reserved for a higher plan, which is not true of several dedicated form tools that charge specifically for this.
The practical use is shortening. Ask what kind of enquiry this is, then show only the fields that apply to that answer. Every field you can hide from someone is a field they do not abandon over, and a form that adapts feels considered rather than bureaucratic.
The one caution is testing. Branching logic multiplies the number of routes through a form, and it is easy to build a path where a required field is never shown, which produces a submit button that silently does nothing. Walk each branch before it goes live.

What happens after submit
| Action | Note |
|---|---|
| Show a custom message | Editable, rather than a fixed thank-you string |
| Redirect to a URL | The usual route to a booking page or a video |
| Trigger a workflow | Where the real value is. Tag, notify, text, assign, enrol |
| Create or update the contact | The submission becomes a CRM contact, which is what makes the workflow above possible |
Compare that with a standalone form tool, where a submission produces an email and then somebody has to do something with it. Here the submission is already a record in the system that will chase the lead, and the difference in response time is the difference between a lead that converts and one that goes cold over a weekend.
The obvious first workflow is an instant text to the enquirer and a notification to whoever handles enquiries. That is two actions on the workflows canvas, and remember that the text itself is billed from the wallet at the rates on the phone system page.
The limits
| If you want | The position |
|---|---|
| Heavy visual design control | Themes plus custom CSS. Fine if you write CSS, limiting if you do not |
| File uploads | Check this against your own use case before relying on it. Test with real file sizes |
| Payment on the form itself | That is an order form or a funnel step rather than a form |
| Typeform-grade polish | Not comparable. Surveys are the nearest thing and they are plainer |
| Forms without the rest of the platform | Wrong purchase. The CRM connection is the entire value here |
The last row is the summary. Judged as a form builder against dedicated tools, this is average. Judged as the front door to a CRM you already pay for, it is the obvious choice, and nobody should be buying a $97 a month platform for its forms.
Forms sit inside the website builder and the funnel builder, and embed anywhere else.
Forms are on every plan
Embedding one on an existing site is the quickest way to see whether the CRM half is worth having, without touching your website.

If forms and a funnel are all you need
Systeme.io includes forms, funnels and email on a free plan with no sub-accounts to configure. Less capable than this, and free, which is the trade.

If the page around the form matters most
ClickFunnels treats the form as part of the page rather than a separate object, and its builder is the best of the three. Weaker CRM behind it, which is the trade.

Common questions
Can I embed GoHighLevel forms on my own website?
Yes, on any site including WordPress, in four layouts: inline, popup, polite slide-in and sticky sidebar. Submissions land in the CRM as contacts and can trigger workflows immediately, so you can use the CRM without migrating your website.
What is the difference between a form and a survey in GoHighLevel?
A form shows every field on one screen, a survey presents them a step at a time. Use a survey once you are asking for more than about six things, because long single-page forms are abandoned far more often than the same questions split across steps.
Does GoHighLevel support conditional logic in forms?
Yes, in both forms and surveys, and it is included at every plan tier rather than being a paid upgrade. Test every branch before going live, since it is easy to create a path where a required field is never shown and submission silently fails.
Can I style GoHighLevel forms to match my brand?
There are built-in themes and support for custom CSS on forms, surveys and quizzes. The themes are enough for most cases and the CSS option is there when they are not, though it does assume someone who can write CSS.
Can a GoHighLevel form take payment?
Not directly. Forms collect information; taking money is the job of an order form or a funnel checkout step, which connects to Stripe. If you need payment and questions together, build it as a funnel rather than a form.
What happens when someone submits a GoHighLevel form?
They become a contact in the CRM, and you can show a custom message, redirect to a URL, and trigger any workflow. The usual setup is an instant text to the enquirer plus a notification to whoever handles enquiries.
